Do they spawn in a certain place, are they triggered by something, or are they rare spawns?

What I was asking (with more elaboration this time) is, when you go to the epaulets section in the cash shop, all of the epaulets sold there say "no visual effect".

nope, just checked in game, it is purify epaulet, not s.tit.
the design is different, and this one gives you a breathing mask. :3
