all the numbers r not exact, there assumptions....... its purely luck and there is no rate catalyst..

its cabal luck. I once failed to slot a catalyst with 16% fail chance twice in a row. Another time slotting osm helms a year ago i slotted rate twice in 10 helms (which i made into amp helms)
