Someone had a rough chart a while ago on eu forums is was something like this:
0-1 100%
1-2 95%
2-3 80%
3-4 70%
4-5 60%
5-6 55%
6-7 50%
7-8 45%
8-9 40%
9-10 35%
10-15 30%
Not sure how accurate it is but seems legit

From my personal experience it feels more like...
0-1 100%
1-2 95%
2-4 80%
5-15 33%

From my experience it feels like:
If you want to fail:
+15 no problem
If you want to succeed
stays at +2 for life
- Retired
- Retired

it gives u a 100% rate from 0 to +6 and stop working. but if u fail it will bring u down ONLY 1 instead of 2 at higher +'s
