I'm not sure if this has been reported, but I'll just say it anyway. This happened to me about 4-5 times already, and I feel like I should report it.
When I'm in a dungeon, and I'm at the part where you kill the Crystal Seals / Soul Lamps, I turn to the one in the bottom left corner. Then, when I cast a skill, like Illusion Stab, it starts normal attacking, about two strikes, then it double casts Illusion Stab and Dance of Ruin at the same time. (these skills are in sequence). So like, is this a bug or like a new skill update?
Well anyways, this is annoying, especially when I can't combo like this...

It's some kind of bug. I heard it was around since open beta.
