It's not a bug

An item that is +6 (maybe it was 7 or 9 i don't remember) requires 2 safeguards in your inventory. ie, you weren't using them the right way...

Safeguards only prevent one downgrade. If the upgrade fails and the item is supposed to go down -2, you will need 2 safeguards. If you only have one safeguard, it will he used amd the item will only go down 1 grade.
